Job Description:
MOBILE CLINIC CLINICIANUnder the general supervision of the
Associate VP of Innovation & Community Engagement, the
Community-Based Mobile Clinic Clinician delivers proactive,
person-centered clinical and preventive services to individuals
across Wayne County. This role emphasizes outreach and engagement
through mobile health, meeting people where they are, with the goal
of increasing equitable access to behavioral health care, reducing
disparities, and promoting mental wellness in underserved
populations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provides direct clinical and preventive behavioral health
services in the community, including crisis intervention,
de-escalation, brief assessments, education, and engagement.
- Submits complete case documentation following DWIHN policies
and procedures.
- Documents all crisis intervention activities in the electronic
medical record to ensure accuracy of records and regulatory
compliance.
- Provides coordination and follow-up for clients and assists in
obtaining necessary additional services, coordinating with the
Access Center or the individual's current outpatient therapy
plan.
- Participates in mobile outreach events and health equity
campaigns focused on serving historically marginalized groups,
including seniors, youth, unhoused individuals, and communities of
color.
- Collaborates with peer support specialists, nurses, outreach
teams, and local partners to ensure integrated service delivery
across physical and behavioral health needs.
- Identifies social determinants of health impacting clients and
connects them to necessary resources such as housing, food,
transportation, and legal supports.
- Utilizes trauma-informed, culturally responsive practices that
honor the lived experiences and identities of individuals
served.
- Engages caregivers and families in care planning and preventive
wellness conversations to support recovery and long-term
stabilization.
- Supports population health efforts by promoting mental health
education, substance use prevention, and resilience-building within
community spaces.
- Maintains timely and complete documentation in the electronic
medical record in accordance with DWIHN and MDHHS standards.
- Participates in community advisory boards, neighborhood events,
and advocacy efforts to elevate community voices in service
delivery. KNOWLEDGE, SKILLS AND ABILITIES (KSA'S)
- Knowledge of crisis training and experience in de-escalation,
i.e., suicide prevention, development of crisis plans, trauma
informed care, etc.
- Strong knowledge of health equity, prevention frameworks,
trauma-informed care, and mobile outreach models.
- Knowledge of local neighborhoods and the unique behavioral
health challenges within underserved Wayne County communities.
- Experience using motivational interviewing and brief
interventions to build trust with people in crisis or disconnected
from traditional systems.
- Ability to navigate community-based settings, including
shelters, encampments, schools, and other non-traditional care
sites.
- Skilled in interdisciplinary collaboration with physical health
providers, housing partners, peer specialists, and law enforcement
as appropriate.
- Proficient in electronic documentation, communication, problem
solving, and crisis decision-making.

REQUIRED EDUCATION:A Master's Degree in Social Work, Psychology,
Counseling, Human Services, the Social Services, or a related
field. REQUIRED EXPERIENCE:One (1) year of full-time paid
professional clinical experience working in a direct service
capacity in community mental health. REQUIRED LICENSE(S).A Valid
State of Michigan clinical licensure: RN, , LMHC, LPC, LLP or PhD.
(A limited license will be accepted).A Valid State of Michigan
Driver's License with a safe and acceptable driving record. WORKING
CONDITIONS:Employees in this classification must travel throughout
Wayne County. Candidates will be required to travel to
community-based settings, hospitals, schools, courts, etc. The work
day is spent on a mobile clinic van. There will be positions
offered for a shift of 10am-6pm. Candidates must be able to work a
flexible schedule.
